The following documents must be completed :
Curriculum vitae : This
must be competed by principal applicant and spouse (please note
that the dates must correspond to the dates on each of your reference
letters). If you already have an updated CV, please provide a copy
of it.
Selection Certificate
of Quebec form : The principal applicant and spouse must both
complete an application.
Selection certificate
of Quebec form : (children 18 years of age or older)
All dependent children of 18 years of age or older, single and still
studying, must all complete a separate application.
Selection certificate
of Quebec : (Appendix Declaration of spouse or de facto spouse)
This declaration must be completed by the principal applicant and
the spouse or de facto spouse.
The following documents must be signed only :
Mandate for Skilled
Workers : To be signed by the principal applicant at the bottom
of the page, to left, where indicated ‘Signature of applicant’.
Power
of attorney : To be signed by the principal applicant, center
page to the right, where indicated ‘’signature’’.
Last page of
Selection certificate of Quebec : To be signed by the principal
applicant and spouse must both sign on page 4. The principal applicant
must sign where indicated ‘’Principal applicant’’
and the spouse must sign where it indicates ‘’ spouse’’.
Last page of
Selection certificate of Quebec for children 18 years of age or
older : The principal applicant and each children of 18 years
of age or older, single and still studying must sign at page 3.
The principal applicant where it indicates ‘’principal
applicant’’ and the children must sign where it indicates
‘’Dependent child’’.
Appendix Declaration
of spouse or de facto spouse :
the principal applicant and the spouse must sign on page 1. If they
are not married and they are facto spouse, then they must on page
2 of the Appendix Declaration of spouse. On page one the principal
application shall signs the declaration where it indicates ‘’signature
of the principal applicant’’ and the spouse must sign
where it indicates ‘’signature of the spouse of the
principal applicant’’. Same thing applies for the facto
spouse on page two.
1. Quebec Processing
Fees:
The first fees to be paid when you submit your application are the
Quebec processing fees.
Fees must be paid by bank draft, in Canadian dollars, drawn on bank
in Canada, and payable to ‘the Receiver General for Canada’.

to our professional fees, and we will paid them on behalf.
2. Our professional fees
Our fees to process your immigration application are totally guaranteed.
The first half is payable when you send us your file and second
half will be payable once you are accepted by the immigration authorities.
In case of refusal, the first payment will be totally refunded.
If you decide to go on with your file, you should send us all the
documents needed, with half of our fees and the processing fees.
You can send the fees and the processing fees by making a bank draft
to Bohbot & Associates or you can make a wire transfer according
to the instructions herein attached.
